Please initial after each statement to indicate your compliance. Any employee in violation of these practices is subject to disciplinary action, up to and including termination. 1. Arrival: Park in the employee parking lot in the gated area. When entering the building on your first day, you must sign the sign-in sheet at the front desk. This is to ensure that, in case of a fire, the safety and rescue team knows you are in the building. After you are issued a badge, you will enter and exit the facility by scanning in and out. * 2. Safety: Safety is everyone’s responsibility. Schwan’s will provide you a safe workspace, but it is your responsibility to keep safe work principles in mind at all times while on the job. You must be safe over everything else. * a. Safety is the most important objective with Schwan’s. Attention to personal and team safety is an absolute requirement * b. Slips / Trips / Falls: Be cautious as you move through the plant. Always walk, use handrails when using stairs, and maintain 3 points of contact. Be sure to wear slip resistant shoes with good traction that are not worn-out. If the floors are wet/icy you must take slow and short steps, improve balance by keeping hands free, and increase center of gravity by keeping body weight directly over feet. * c. Employees will be trained in the safe and proper techniques for completing tasks. Any safety concern due to a deviation from any trained process may result in disciplinary action up to and including termination. * d. Safety glasses, slip resistant shoes, bump caps, and ear plugs must be worn at all times within the production facility. Any employee found in violation of this policy will be disciplined up to and including termination. * Horseplay of any type will NOT be tolerated * f. Lock-out Tag-out (LOTO): Lock-out tag-out prevents injury from the unexpected start-up of machinery during servicing and maintenance. Failure to follow Lock-out Tag-out procedures will result in serious injury. You will not be allowed to lock out a machine without first conducting Schwan’s Authorized LOTO training. * g. Machine Safety: Our process equipment has many moving parts. Never reach into any running equipment. We never operate machinery without guards in place. Machine guards and safety gates are to be kept in place at all times while production is taking place. Do NOT reach past any gates or guards. * h. Anhydrous Ammonia: Anhydrous Ammonia is in use as a refrigerant in cooling in the facility. The facilities are covered by Process Safety Management. In an emergency, avoid contact with Ammonia and exit upwind. Familiarize yourself with exit locations and assembly areas within our facility. * i. Forklifts should only be operated by those who have been licensed and certified. In the event your job duty requires operating a forklift, Schwan’s will properly train and certify you on its use. Pedestrians should always be alert for lift traffic. Watch and listen at all times. DO NOT walk under a lift. Maintain a safe distance from any raised lift. Before crossing the path of a lift, make sure the driver sees and acknowledges you before you proceed. * One of the leading causes of back injury in the workplace is an improper lifting technique. Use proper lifting techniques to help you avoid these very painful injuries. Size up the load before trying to lift it. Bend your knees. Allow your legs, not your back, to do the work. Carry the load close to your body. Do not twist or turn your body once you have made the lift. Make sure you can carry the load where you need to go before attempting to move it. Make sure your path is free of obstacles and there are no hazards such as spills in your path. Set the load down properly. Lower your body slowly, letting your legs do most of the work. Always push, not pull an object. Pushing puts less strain on the back. * 3. Personal Protective Equipment: WHITE hairnets, safety glasses, beard nets, earplugs, gloves, and smocks will be provided by Schwan’s. You must wear all PPE as required. Do not remove the cord from the ear plugs. Smocks are ONLY to be worn in the production area. Gloves must be worn anytime you are handling product. If using reusable gloves, you must wash with soap and water each time you return from breaks or when you touch an unclean surface. You will be responsible for wearing slip resistant shoes for any assignment at Schwan’s. Any other PPE required for a specific job will be provided by Schwan’s and needs to be returned at the end of the assignment * o Confirmation that employee owns or is arranging to purchase slip resistant shoes. * o ALWAYS walk through the foot baths. Never jump or step over. * 4. Attendance: A very high attendance standard is set for all employees assigned to Schwan’s. Employees who MUST miss work need to contact 1st Employment no later than 1 hour prior to the beginning of shift. ABSENCES and/or TARDINESS will not be tolerated. If at any time this becomes an issue, your assignment will be terminated immediately * o Job Abandonment Policy: Abandoning or not completing a shift WILL cause your pay to be dropped down to minimum wage for all hours worked during the week of abandonment * o Tardiness: You must contact 1st Employment if you are running late for your shift. If you are assigned to report in at 6:30 am, this means you need to be clocked in at 6:30 am and on your line or designated area promptly after getting your PPE and washing your hands. * o No Call No Shows: Failing to contact 1st Employment at least 1 hour prior to the beginning of your shift will be considered as a no call, no show. This will result in immediate termination from 1st Employment. * 5. Deductions: You must sign a Payroll Deduction Form for any additional PPE given by Schwan’s for any lost or forgotten items. This will include safety glasses, badges, etc. * 6. Time Clock: The Schwan’s timeclock is located in the hallway that leads to the breakroom and production area. A badge will be provided to you for you to clock in and out. Clocking in and clocking out, is YOUR responsibility. Please ensure that your card swipe reads accepted. If you have any issues or questions, please contact your Supervisor as well as your 1st Employment Representative as soon as possible. 7. Fire exits: Supervisors will make you aware of fire exits. Please make a note of where these are located. Never block aisles around fire extinguisher and fire exits * EATING / DRINKING: Eating and drinking are allowed only in the break rooms and office areas. Drinking water outside of the lunch room is allowed at designated drinking fountains. *Remember that hands must be washed after eating and drinking * HEARING PROTECTION: Any hearing protective devices must be corded or hearing muff style. * 8. Breaks/Lunch: You will receive a 30 minute lunch break. You may leave the facility, however you must clock in and clock out. You must not leave your work station until your designated lunch time and return promptly after. In addition to the lunch break, employees will receive 2 ten minute paid breaks * GARMENTS / FOOTWEAR: Must wear clean outer garments. Only pants or jeans that extend to the ankle are allowed. Socks have to meet the pants so there isn’t any exposed skin. Clean, closed toed shoes must be worn * PERSONAL ITEMS: No personal items in production areas (coats, and lotions, keys, etc.) No cell phones are allowed in production areas without management’s approval. Employees needing personal medications must be pre-approved by Human Resources * 9. Housekeeping: Properly dispose of all trash and debris. Should you see anything on the floor, please pick it up to reduce trip hazards. Keep all aisles, walkways and exits clear of obstacles. Outside and in the break area, please dispose of all trash in the proper receptacles. * GLASS: No glass, ceramics, or brittle plastic in the plant at any time * CONSTRUCTION OPERATION: Any cutting, drilling, etc. construction operation must be shielded from food materials, food contact surfaces, and/or food containers to avoid foreign material contamination. Construction equipment will not be stored on top of or intermingled with production equipment. If any construction activities have created conditions that require clean up prior to manufacturing start up, Schwan’s on-site management must be notified. * 10. Tobacco: Smoking and use of other tobacco products are not allowed on Schwan’s property * CONTACT: Sneezing or coughing on food, food contact equipment or packaging materials is prohibited. No sitting, standing, or walking on raw material or food contact surfaces. * DOORS / WINDOWS: All doors and windows when not in use must be kept closed to prevent entry by pests and vermin. * 11. Dress code: Employee must show up to work wearing job appropriate clothing. Any violation of this could result in being sent home or dismissal. No hoodies, tank top, mid-drift shirts, No cut off sleeves, hooded clothing, and spaghetti straps shirts are not allowed as well as any violent, sexual, drug/alcohol related, profanity, obscenities or any other printed designs inconsistent with the Discrimination & Harassment Prevention Policy. * ALLERGENS: Allergens are used in the plant. If you have concerns regarding this plant’s allergens, please contact the plant FSQ manager * INFECTIOUS DISEASES: At this time if you are suffering from, or have you come in contact with any INFECTIOUS, DISEASES, or other HEALTH CONDITIONS you should not enter the facility. If you are feeling ill today, please contact the plant FSQ Manager before entering the facility * 12. Colognes/Perfumes: Many have perfume allergies, so please be respectful of this. Keep fragrances muted. Good hygiene is a must, but do not wear strong perfumes/colognes. * 13. Language: Only use work appropriate language. Obscenities or offensive language will not be tolerated. * 14. Attitude: Employee must exhibit a professional appearance and demeanor at all times. Also must have a positive work attitude and show motivation * 15. Lockers: Lockers are available upon request to supervisor * 16. Weapons: Weapons of any kind are prohibited on company pro * 17. Cell phones: Cell phones must be turned off during work hours. Sending/receiving text messages or making/receiving calls during work hours is prohibited. You are only allowed to use your cellphone in your break/lunch time and only in the break room or outside the building. Inappropriate use of your cellphone could result in disciplinary action up to and including termination of employment. * 18. Update 1st Employment: It’s important you keep 1st Employment informed at all times on any changes in your assignment (e.g. shift and/or cell). Also, please let 1st Employment know if your contact number changes as a 1st Employment Representative may call you to let you know if a line is down so that you are not making an unnecessary trip to work. Remember, 1st Employment’s office number is (479) 238-8989 and the Emergency Line is (479) 228-1525. * 19. It is very important that 1st Employment be advised immediately of all information about a work related injury. 1st Employment’s policy requires that you notify both your On-Site Supervisor and your 1st Employment Representative of your injury. If circumstances don’t allow an immediate notification, you must report the incident to 1st Employment as soon as practicable. 1st Employment has a 24-hour a day, seven-day a week mobile app, emergency cell phone 479-238-8989 and email service, workcomp@1stemployment.net, to take your report. It’s your responsibility as our employee to report your injury to 1st Employment prior to receiving any medical treatment otherwise WC will not cover. The customer is NOT responsible for reporting your incident/accident nor authorized to approve treatment. Any person who knowingly presents a false or fraudulent claim for the payment of a loss is guilty of a Class D felony, and may be subject to maximum imprisonment in the state prison for up to six (6) years, or by a fine of up to $10,000, or by both. In such cases, the Insurance Commissioner will refer the matter to the local prosecuting attorney. * 20. Drug Screens: 1st Employment Staffing and Schwan’s are proud to work together in keeping a workplace free of drugs. 1st Employment Staffing may perform pre-employment drug screens, post-accident, random and reasonable suspicion drug screens or alcohol test. No employee will be allowed to work under the influence of alcohol or any controlled substance or allowed to bring alcohol or any controlled substances to company property * 21. Checks: Employee’s have the option to setup direct deposit to your checking or savings account or direct deposit to a global cash card which is available at the 1st Employment office. If neither option is selected, checks will be mailed out from the corporate office on Friday afternoons for all hours worked the previous week. Once checks have been mailed out, we have no control over when they will arrive at your address so we highly suggest setting up direct deposit * 22. Job Changes: In order to ensure your safety, it is very important that 1st Employment be advised immediately of any changes to your job title, department and/or primary job duties and tasks. In addition, you should never perform a job that you have not been properly trained and authorized to perform * 23. Soreness versus Sprains/Strains: It is very common to experience muscle soreness and/or stiffness when muscles are exposed to new or repetitive movements. The presence of muscle soreness and/or stiffness does not mean you have injured yourself, it is simply a side effect of physical activity your body isn’t used to. With muscle soreness, you typically won't feel it until the following day or even 1-2 days later. With a strained muscle, the pain is usually immediate, sharp/intense, and localized to where the injury occurred. Stretching before and after your shift, staying hydrated and using heat or ice on sore muscles following your shift can help. If you are in considerable pain, please report this to 1st Employment immediately. * FOOD SAFETY AND QUALITY HAIR NETS / BEARD NETS: All personnel when in the plant must wear a hood-styled hair net and/or the designated color hair net (including in the restrooms and breakrooms). A new hood-style hair net will be used upon reentry to the production facility. Any facial hair must be fully covered by the hood. * HAND CARE: Hands must be washed with soap and warm water after each use of the toilet facilities and also before handling and product or ingredient on the line. Hand washing is approx. 20 seconds of hands under the water with anti-bacterial soap. * JEWELRY: No jewelry is allowed. Exceptions: Plain wedding bands that cannot be removed are allowed but must be taped and gloved. Medical alert necklaces/bracelets are allowed, however your Supervisor must be notified. * NAIL POLISH/PERFUMES: Fingernails shall be kept short and clean. Nail polish and false fingernails may not be worn in the plant. No use of perfumes or colognes. * SMOCKS: All personnel in GMP areas are required to wear a company issued smock. Smocks must be removed before entering restrooms, break rooms, or exiting the building. * GUM CHEWING / CANDY: Chewing gum / candy is not allowed in the plant * TOBACCO PRODUCTS: Smoking and the use of other tobacco products (Snuff, Chewing, and Electronic Cigarettes) are not allowed on Schwan’s property. * EATING / DRINKING: Eating and drinking are allowed only in the break rooms and office areas.
